The Department of State inspector general has determined former Secretary of State Hillary Clinton mishandled emails and failed to properly respond to cybersecurity risks during her tenure.

The findings of the department’s 78-page document are the result of an audit performed by an internal investigator, according to the Associated Press. Clinton and several of her predecessors were faulted in the document released Wednesday. The investigation was launched after it was revealed Clinton utilized a private email account and server while performing her duties as secretary of state.

The report stated that Clinton, her predecessors and the department itself was “slow to recognize and to manage effectively the legal requirements and cybersecurity risks associated with electronic data communications, particularly as those risks pertain to its most senior leadership.”

Despite the report noting that it found “longstanding, systemic weaknesses” in the department, Clinton’s indiscretions were singled out as particularly “more serious,” according to the Associated Press.

The report follows the announcement that the hacker known as “Guccifer,” who claims to have breached Clinton’s server, is in the midst of finalizing a plea deal with the U.S. government. He is scheduled to appear in court Wednesday.
